The lead at the top has been cut to just six points for Leeds while the Baggies are a point further ahead.
Marcelo Bielsa’s side were beaten 2-0 by Sheffield Wednesday in their Yorkshire derby at Elland Road as Gareth Southgate watched on from the stands.
Wednesday had to soak up plenty of pressure in the first half as Leeds dominated the contest.
But former Leeds boss Monk saw the Owls improve after the break and Jacob Murphy gave them the lead three minutes from time.
Substitute Atde Nuhiu wrapped up the win in the fourth minute of stoppage time to take Monk’s side into the play-off places.
West Brom were held at The Valley by Charlton in the second meeting between the sides in the space of a week.
Kenneth Zohore opened the scoring after 22 minutes for the Baggies before Josh Davison quickly equalised for the Addicks.
Less than 60 seconds after the break, Hal Robson-Kanu restored their lead but defender Tom Lockyer earned Lee Boywer’s side a point.
Ivan Cavaleiro scored the only goal of the game at the KCOM Stadium as Fulham beat Hull City 1-0 to take advantage of the top two’s slip up.
In Saturday’s early kick-off, Brentford beat their local rivals QPR 3-1 in the final derby to be played at Griffin Park.
There was late drama at the Madejski Stadium as Reading and Nottingham Forest both scored in added time.
